We start this week’s Age UK Network News Roundup at Age UK Croydon, who have received a positive independent evaluation of their Personal Independence Coordinator Service. Elsewhere there’s the usual list of vacancies, with two new open roles at Age UK South Gloucestershire, four at Age UK Nottingham & Nottinghamshire, and a new Advice & Information Worker position at Age UK Redbridge, Barking & Havering.

Independent Evaluation of Age UK Croydon’s Personal Independence Coordinator Service

Age UK Croydon has recently seen their Personal Independence Coordinator Service (PIC) receive an independent evaluation including analysis from interviews with key health and social care partners; NHS friends and family surveys; and a contributory survey by Healthwatch Croydon.

The summary highlights a significant and often transformative impact on the health and wellbeing of PIC service clients and outlines key learnings to inform the delivery of primary care and commissioning going forward.

“We proudly welcome the release of the independent evaluation of Age UK Croydon’s Personal Independence Coordinator Service (PIC). Commissioned by Age UK Croydon, and funded by NHS South West London, the evaluation combines analysis from interviews with key health and social care partners, NHS friends and family surveys, and a contributory survey by Healthwatch Croydon.

The Age UK Croydon Personal Independence Coordinators Service (PICS) provides person-centred care coordination, informal emotional support, and advocacy for people over 50 across Croydon. The service has worked with more than 3,000 clients since its inception, and is funded by South West London NHS, and forms a key part of the One Croydon Alliance Integrated Care Network model (ICN), a partnership between health, social care, and voluntary sector services. It is part of a move towards a more person-centred, holistic view of patient care, and to reduce the impact of non-medical issues on healthcare. The service is based in all 48 GP practices across the Borough and within the ICN+ Multi-Disciplinary Teams.

PICs are proving to bridge the gap sometimes experienced between health and care professionals and clients in the community by working closely with clients to identify and work towards goals to improve their quality of life.

The summary highlights a significant and often transformative impact on the health and wellbeing of PIC service clients and outlines key learnings to inform the delivery of primary care and commissioning going forward.  

Key Outputs:

1,127 clients accessed the service in the last year, 1,382 goals were set in the last year August 2020 – September 2021, 18,507 contacts took place2

Key Outcomes:

98% of clients improved their mental wellbeing in the last year (94% over the last 3 years) 99.6% of clients achieved at least one of their wellbeing goals (in the last year) 83% of all wellbeing goals were achieved (in the last year)

Two open roles at Age UK South Gloucestershire

Age UK South Gloucestershire are looking to hire two new staff members: A Walking Project Coordinator to help reintroduce physical activity into the daily lives of older people in the area, and a Digital Outreach Worker to tackle digital exclusion.

Several Vacancies at Age UK Cheshire East

Age UK Cheshire East currently has several vacancies available, including Drivers, Home Helpers, and a Dementia Activity Worker.

The Dementia Activity Worker will support people living with dementia to maintain their skills, to benefit from activities, company and friendship; and to have some fun. You will receive training in Maintenance Cognitive Stimulation Therapy and you will help to plan and deliver a programme of activities in their day centre in Macclesfield and some outreach sessions across the eastern part of Cheshire East.

Four vacancies at Age UK Nottingham and Nottinghamshire

Age UK Nottingham and Nottinghamshire have four open roles currently available. They’re looking for a Home Energy Advisor to make home visits across the city, as well as an Operational Manager for Housing in their Safe & Sound Team.

They are also seeking a Charity Support Worker to help unpaid carers to access their rights and entitlements, and there is also a vacancy for a Mini-Bus Driver.

Three open roles at Age UK Redbridge, Barking and Havering

Age UK Redbridge, Barking and Havering are looking for a Senior Manager (Engagement, Facilities and Day Opportunities) to manage and develop a range of services that are responsive to the needs of older people living in a multi-cultural borough.

They are also seeking a Digital Outreach Worker to facilitate outreach visits to promote referrals to the Digital Inclusion Service for older people across Redbridge, Barking and Havering and enable participation of clients who are unwilling or unable to attend volunteer-led sessions.

There is also a vacancy for a Advice & Information Worker to support the Advice & Information service across Havering.
